If your child needs medical, dental, hospital or other health services, the parent must give permission in most cases.  It's the law.  The only time medical care may be given to a child without the consent is in a life- or limb-threatening situation.

IC 16-36-1-6 provides a parent, authorized to consent to healthcare for a child, may delegate the authority to another person for a period of time that the parent will not be reasonably available to exercise the authority.  To help you comply with the law, Ball Memorial Hospital has developed a Medical Information Consent Form for Children, which can be downloaded from the link on this page.

Whenever you're going away, whether it's for two hours or two weeks, across town or 1,000 miles away, make sure the person caring for your child has a written record of his or her health information.
