Determining Whether Your Insurance Will Cover Surgery

• Contact your insurance company and have them explain how they cover bariatric services
• Documents that might be needed by your insurance company:
  • Documentation that verifies previous attempt at weight loss such as Weight Watchers records
  • Nutritional Counseling records
  • Referral from primary care physician
• If your insurance will not cover the surgery, you do have the option to pay for the surgery yourself.  Information on this process is available upon request
